  3. Sign the form in the spaces provided on pages 3 and 5.
  4. Submit the form to the Human Resources Department.

  1. Download the file to your computer.
  2. Sign the City employment application form in the spaces provided on pages 3 and 5; sign any other documents, as indicated, and have your personal physician fill out and sign the Physician Statement.
  3. Submit the forms and all related documents (details in packet) to the Human Resources Department.
